Before your appointment

We want to make your time with us as efficient and pleasant as possible. Completing the following steps before you arrive is essential.

Step 1 - Look for the text message from Klara. Select an option to - Confirm, Reschedule, or Cancel.

Step 2 - Once confirmed you will receive a 2nd text outlining all of the required information. Complete your medical history either on the EMA patient portal website (see below for link) or download the APPatient App and complete the information on the app. Make sure to complete your medications, pharmacy, allergies, past medical history (including surgeries, GYN history, OB history, family history).

Step 3 - Sign the required forms on the Klara text message. Upload a copy of your ID and insurance card.

Please note the specific browsers needed in order to use the portal online: Google Chrome v90 or newer / Mozilla Firefox v88 or newer.

Please note for APPatient App - iPhones need to be updated to iOS 18 or newer. Android phones need to be updated to Android 7.0 and higher.
